You end the task confused

I have been working on my wooden building together with a hired hand, and after 2-3 days' work, after making a log, I got the message "You end the task confused and seem to have misplaced something". What happened?

I cannot figure out what I have misplaced (fortunately none of my precious masterwork axes, nor the masterwork longbow, nor my silver ring). Also, why did this happen? I was not all that tired at that point.

Usually it’s a tool of sorts. Handaxe or knife , maybe other utility items can go missing too.

I guess it’s similar to getting Influenza; randomizer throwing “mishap” your way.

Thanks! Is the item properly gone, or is it suppose to still be somewhere nearby?

AFAIK: It is gone.
At least I’ve never managed to find the missing item. Often it’s a handaxe in the early game, you’re moving on world map and then you get the “it’s gone” 
Zooming in, looking around and backtracking to previous wilderness tile never has materialized the missing item.

Wow. That never happened to me. Didn't know it's possible at all!
